CTLA-4, also known as cytotoxic T-lymphocyte-associated protein 4, or CD152 (cluster of differentiation 152), is a protein receptor that plays a role in downregulating the immune system. It is a member of the immunoglobulin superfamily and is expressed on the surface of helper T cells, where it transmits inhibitory signals to T cells. The protein consists of an extracellular V domain, a transmembrane domain, and a cytoplasmic tail. There are alternate splice variants that encode different isoforms. CTLA-4 is structurally similar to the co-stimulatory protein CD28, and both molecules bind to CD80 and CD86 (also known as B7-1 and B7-2) on antigen-presenting cells. While CD28 transmits stimulatory signals, CTLA-4 delivers inhibitory signals to T cells. CTLA-4 is also present intracellularly in regulatory T cells, where it may play a crucial role in their function. The expression of CTLA-4 increases in T cells following activation through the T cell receptor and CD28. CTLA-4 acts as an inhibitory receptor for B7 molecules. Clinical trials have explored the use of a fusion protein of CTLA-4 and antibodies (CTLA4-Ig) for rheumatoid arthritis treatment.
